Adolygiad o'r Berthynas rhwng Iechyd a Gofal CymdeithasolMae Pwyllgor Iechyd a Gwasanaethau Cymdeithasol Cynulliad Cenedlaethol Cymru yn cynnal adolygiad o ba mor effeithiol yw trefniadau'r gwasanaethau iechyd a gofal cymdeithasol o safbwynt cydgynllunio a darparu'r gwasanaethau hyn. Rwy'n ysgrifennu atoch i'ch gwahodd i gyflwyno tystiolaeth ysgrifenedig gerbron y Pwyllgor.Nodau'r adolygiad yw pwyso a mesur pa mor effeithiol yw'r ffordd y mae'r gwasanaethau iechyd a gofal cymdeithasol yn cydweithio a chydweithredu â'i gilydd; nodi arferion da a chynnig argymhellion er mwyn annog y prif asiantaethau i wella'r broses o gydweithio a chydweithredu.Dyma gylch gorchwyl yr adolygiad: i. Adolygu'r dulliau o gydgynllunio a darparu gwasanaethau ym maes iechyd a gofal cymdeithasol, ac ansawdd y sylfaen dystiolaeth; ii. gweld pa mor atebol yw'r trefniadau ar gyfer cydgynllunio a darparu gwasanaethau;iii. cloriannu'r effeithiau (cadarnhaol a negyddol) y gall y penderfyniadau a wneir mewn un gwasanaeth eu cael ar wasanaeth arall;iv. edrych ar faterion allweddol sy'n effeithio ar ansawdd gwasanaeth cydgysylltiedig a'r ffordd y caiff ei ddarparu, yn benodol:
  • y trefniadau ar gyfer rhyddhau cleifion o'r ysbyty
  • gofal canolraddol
  • gwasanaethau preswyl a gwasanaethau cartrefi nyrsio
  • gwasanaethau gofal yn y cartref
  • y rhan y mae'r sectorau annibynnol a phreifat yn ei chwarae
  • cymorth i ofalwyr.
v. adolygu'r rhan y mae gwasanaethau iechyd a gofal cymdeithasol yn ei chwarae wrth annog cleifion i fod yn annibynnol ac wrth eu hatal rhag cael eu derbyn neu eu derbyn drachefn i'r ysbyty yn ddi-angen.Os hoffech gyflwyno tystiolaeth, a fyddech cystal â'i hanfon, drwy'r e-bost neu ar ddisg os yn bosibl, ond fel arall, ar gopi caled, at Claire Morris, Dirprwy Glerc y Cyngor, y Pwyllgor Iechyd a Gwasanaethau Cymdeithasol, Ysgrifenyddiaeth y Pwyllgor, Cynulliad Cenedlaethol Cymru, Bae Caerdydd, CF99 1NA. Efallai na fydd yn bosibl i ni ystyried unrhyw ymatebion sy'n dod i law ar ôl y dyddiad hwn.Wrth baratoi eich cyflwyniad, a fyddech cystal ag ystyried y canlynol:
  • Dylai'r dystiolaeth fod yn ffeithiol yn hytrach nag yn anecdotaidd.
  • Dylai'ch ymateb fod mor gryno â phosibl.
  • Fel rheol, mae'r Cynulliad Cenedlaethol yn cyhoeddi ymatebion i brosesau ymgynghori cyhoeddus, ac mae'n bosibl y cânt eu gweld hefyd gan Aelodau'r Cynulliad, a'u trafod mewn cyfarfodydd Pwyllgor. Os nad ydych am i ni gyhoeddi eich ymateb na'ch enw, mae'n bwysig eich bod yn nodi hyn ar ddiwedd eich cyflwyniad.
  • Byddwch cystal â nodi a ydych yn ymateb ar ran sefydliad, neu fel unigolyn.
  • Nodwch hefyd a fyddech yn barod i gyflwyno tystiolaeth lafar gerbron y Pwyllgor.
